Setting up options in a custom form

Hi,


Maybe a need a stronger coffee but I just can't figure this out. :huh:



I need to create a custom form that looks like a surveymonkey questionnaire - see the attached example.



The layout of a "tickbox table" is close to what I need but this question type allows multiple selections.



I'll have to use the "option list" type but I can't figure out how to align them horizontally…



Any help would be greatly appreciated!!

surely a bit of css trickery could get your layout reasonably close to that? You might need to use a bodycopy for the page contents to get the look you desire though.
