Custom form Submission records via TRIM records

I have done everything from my end to get this logs reports via TRIM. It’s not working.

I have followed the squiz notes. but I get some thing error on my error report.

please note the error below

(/packages/trim/lib/trim_common.inc:1197) - Unable to create new record. No Uri to fetch for injection (TRIM Connectivity Toolkit unique id = 1000248))

No idea what that error means. Probably best to send a support ticket into Squiz so they can log in to your system and have a closer look.
