We are following instructions from Squiz to upgrade our dev server via this path..
4.16.1 -> 4.16.2 -> 4.18.0 -> 4.18.5
On step_02.php we just get step_02 completed rather than the streaming output on the console. When we get to running step_03.php on version 4.18.0 we get the following error..

We've read the similar topics already posted here but can't see that any of the fixes would work in this instance (e.g. we stop apache while running the scripts).
