How effective is Matrix as a Document management system?
I'm sure on a smaller scale it would be good, what if there are a large number of docs?

Effective is a subjective term -- it depends on what your critical success factors for document management are. Matrix can certainly store and index documents, but it only has a web interface, so your users would have to manually upload/download documents. Most document management systems will integrate with client-side productivity suites so that opening/saving documents is seamless.
On a small scale, its quite usable (especially in an extranet environment where you're exposing documents to non-internal users), but on a large-scale you'd probably want specific document management software.
